Surgical Challenges of Meningiomas with Extracranial Invasion: A Clinical Study

Objective: Meningiomas are mostly benign tumors located intracranially and intradurally, but they can rarely show extradural extracranial growth. Surgical treatment of meningiomas is specifically challenging. In this study, we present an illustrated surgical series to convey our experience in treating with extension.
 Methods: We retrospectively reviewed 11 patients meningioma who underwent surgery between 2008 2020. these patients, both intracranial components were confirmed radiologically intraoperatively.
 Results: The 7 males 4 females a mean age 55.4 years. Most presented facial disfigurement or asymmetrical growth their skull. most common symptom at presentation was headache. location the frontal region that paranasal sinuses parietal bone invasion. recognized two distinct modalities destruction: hyperostosis (n=3) osteolysis (n=8). Pathological study revealed atypical features 6 patients. Preop embolization attempted proved difficult. Proper achieved only one patient. commonly encountered challenges large calvarial cranial base defects due erosion, dural defects, managing superior sagittal sinus tumors. Excessive blood particular concern managed by simple scalp clips, intraoperative transfusion, conservative approach for tumor extensions into sinuses. No perioperative mortality occurred. Postoperative CSF fistulas beneath complications conservatively pressure wrapping. fistula developed. Calvarial reconstruction performed PMMA cement where needed.
 Conclusions: extension surgically challenging treatable It contains fine neurosurgical trics its follow-up.
